Prostate Problems and What They Mean

There are two broad categories of prostate problems that all men need to be aware of; Benign prostate hyperplasia, which is an enlargement of the prostate that occurs with age or because of the effects of prostatitis, and prostate cancer. Although younger men can be affected by such problems, prostate issues for the majority of men do not appear until the age of 50 and older.

In men the prostate is a walnut sized gland that encircles the neck of the bladder and urethra, which is the tube that carries urine from the bladder. The prostate is an exocrine gland with the main function of producing fluid that carries sperm during ejaculation. It also helps to control the flow of urine as it passes from the bladder to the penis.

As men age they become more susceptible to prostate problems. The first is benign prostate hyperplasia (BPH) or enlargement of the prostate. BPH is so common that some physicians consider it a normal consequence of aging in males. It results in the enlargement of the prostate, with it alternately pressing up against the urethra and bladder. BPH can also leads to sexual dysfunction; however modern medicine is still unable to understand why. BPH will affect an astonishing 50% of men during their lifetimes but rarely causes symptoms before age 40, with more than half of men in their sixties and as many as 90 percent in their seventies and eighties encountering some of its symptoms. BPH is the most common non-cancerous prostate problem.

Symptoms of Benign Prostate Hyperplasia can include difficulty urinating, waking up several times a night to urinate, frequent urination, inability to empty the bladder, uncontrolled dribbling, painful urination, and painful ejaculation.

Prostatitis, which is an inflammation or infection of the prostate, can affect any adult male but is most common in men, aged 20–50. It is usually caused by a bacterial infection that has spread from another part of the body and is typically treated with antibiotics to fight the infection. Prostatitis can cause difficulty urinating, pain, fever, and blood in the urine.

Prostate cancer is the second most common cancer experienced by men and is the most common cancer in men over 50. The risk of its occurrence increases with age; about 70 percent of all cases of the disease are diagnosed in men age 65 and older. The symptoms of prostate cancer are similar to other prostate problems, particularly difficulty in passing urine, but other symptoms include lower back pain, pain in the hips or pelvis and erection problems. Every year prostate cancer kills thousands of men despite the fact that it can be effectively treated if caught in its early stages.

Even though prostate problems can be embarrassing for some men it is important to see a doctor at the first signs of any symptoms. Regular doctor's visits are also important particularly upon reaching the age of 40. More often then not any problem with the prostate is nothing to worry about and can be treated quickly and easily, but the risks of not getting frequent checkups is clearly not worth what could happen.

• Keep warm. Colder temperatures can cause urine retention and increase your urgency to urinate.

What is prostate cancer?

Prostate Cancer develops by the growth of cancerous cells within the prostate gland. In its advanced form, it can produce the same symptoms as BPH. (Benign Prostatic Hyperplasia)

Warning Signs

Early prostate cancer often does not cause symptoms. When symptoms of prostate cancer do occur, they may include some of the following problems:

A need to urinate frequently, especially at night; Difficulty starting urination or holding back urine; Inability to urinate; Weak or interrupted flow of urine; Painful or burning urination; Painful ejaculation; Blood in urine or semen; and/or Frequent pain or stiffness in the lower back, hips, or upper thighs.

Any of these symptoms may be caused by cancer or by other, less serious health problems, such as BPH or an infection (Prostatitis). Only a doctor can tell the cause. These are two other main prostate problems

1) BPH - (Benign Prostatic Hyperplasia) is the abnormal growth of benign prostate cells. In an enlarged prostate , the prostate grows larger and pushes against the urethra and bladder, blocking the normal flow of urine. BPH is often the result of prostatitis.

2) Prostatitis refers to prostate inflammation and infection, which can be brief or long-lasting, mild or severe symptoms can include frequent, urgent and painful urination, erectile problems or pain >during ejaculation.

Prostatitis is rampant in the US, affecting at least 80% of American men. Most is without major symptoms. Prostate enlargement, called Benign Prostatic Hypertrophy (BPH) is often the result of prostatitis.

Treatment for Prostate Cancer

The treatment given for prostate cancer depends on how advanced or widespread the disease is. If the cancer is small and only present within the prostate gland, it is possible to treat it with surgery (radical prostatectomy) radiotherapy (either external beam radiotherapy or brachytherapy) hormone therapy

Are You Worried About An Enlarged Prostate Or Other Prostate Problem?

In older men, the inner part of the prostate around the urethra may continue to grow, a condition called benign prostatic hyperplasia (BPH) - also referred to as an enlarged or swollen prostate. Perhaps a fourth of men have some degree of hyperplasia by the time they reach their fifties. By the eighth decade, over 90% of males will have prostatic hyperplasia (according to the American Urological Association).

However, in only a small number of cases (about 10%) will this hyperplasia be symptomatic and severe enough to require any surgical or medical therapy. The mechanism for hyperplasia may be related to an accumulation of dihydrotestosterone in the prostate, which then binds to nuclear hormone receptors which then trigger the growth. The normal prostate gland weighs 20 to 30 gm, but most with nodular hyperplasia can weigh as much as 50 to 100 gm. And nodular hyperplasia is NOT a precursor to cancer. A mildly increased PSA (4 to 10 ng/mL) in a patient with a very large prostate can be due to either nodular hyperplasia, or to prostatitis, rather than cancer.

It's good to know that BPH is a non-cancerous enlargement that is very common in men as they age. While this can cause problems with urinating because it can press on the urethra (the tube that carries the urine), BPH is not cancer.

The class of drugs usually used to conventionally treat BPH are the alpha 1-adrenoreceptors, including prazosin, alfuzosin, indoramin, terazosin, doxazosin, and tamsulosin.

Both BPH and prostate cancer, however, can cause similar problems in older men. The "free PSA test" may help determine the difference between BPH and prostate cancer. Prostatitis, BPH, prostate cancer and metastasis are all very common ailments. Many of these symptoms, however, can also be caused by other conditions, such as a urinary tract infection (UTI), acute prostatitis, or even a sexually transmitted disease.

Doctors must determine if a patient's symptoms are due to prostate cancer, BPH, or to a number of other non-cancer-related conditions. Your doctor conducts a digital rectal exam in order to diagnose BPH. Treatment choices for BPH also include an ever watchful waiting.

The Prostate Basics

Basic Facts about the Prostate Gland: The prostate is a sex gland in males. It is around the size of a walnut, and encircles the neck of the bladder and urethra, the tube that expels urine from the bladder. It is partly glandular and partly muscular, with ducts opening into the prostatic portion of the urethra. The prostate is made up of three lobes: a centrally located lobe with one lobe on each side. The prostate gland secretes a slightly alkaline fluid that forms part of the seminal fluid, a fluid that carries sperm during ejaculation.

There are several benign prostate problems that develop in men.
Types of non-cancerous prostate problems, or clinical conditions of the prostate gland that are not cancer, including infections, inflammations, benign prostatic hyperplasia (BPH) - an enlarged prostate. These problems are quite common and may happen to men of all ages.
Specific Non-Cancerous Prostate Problems include prostatism - this term describes any condition of the prostate that causes interference with the flow of urine from the bladder.

Prostatitis - an inflamed condition of the prostate gland. It can be accompanied by pain, discomfort, frequent or infrequent urination, and sometimes a low-grade fever.

Prostatalgia - a term that indicates pain in the prostate gland.

Benign prostatic hyperplasia(BPH)- the condition of an enlarged prostate. BPH is the main non-cancerous prostate problem. It can cause discomfort and create problems urinating. Although it is not cancer, BPH symptoms are very similar to those of prostate cancer. These include impotence, or the inability to have or keep an erection, and urinary incontinence, or the loss of bladder control.

The fear of having prostate cancer can be devastating to most men. Prostate cancer is most successfully treated when discovered early. Consider these statistics supplied from the American Cancer Society: Nearly 80 percent of all prostate cancers is discovered while they are still localized, or confined to the prostate. The five-year survival rate for men diagnosed with prostate tumors that are discovered at this early stage is a whopping 100 percent. Testing works!

In the past 20 years, the survival rate for all stages of prostate cancer has risen due to early detection and treatment. Early prostate cancer often does not present any symptoms and can only be found with regular prostate examinations by your doctor. These tests can help detect, or rule out, prostate cancer. Check back with your physician if you have had an unusual DRE (digital rectal exam), or if your PSA (prostate-specific antigen) level is high. Your physician may order additional tests or suggest repeating the PSA tests if warranted.
