Mycoplasma genitalium is a sexually transmitted infection:
screening, testing, and treatment

Abstract
Mycoplasma genitalium is a sexually transmitted pathogen and have proved on DNA typing showing sequence identical among partners. Screening, testing and treatment of patients is not fully certain because it is very difficult to isolate the pathogens. We don's know the tests most sensitive and most specific, the type of sample, the choice of treatment for the best benefits of patients and how to treat sexual partners.

Mycoplasma genitalium is a stealth bacteria and is very difficult in researc, the organism is difficult to culture, and takes several weeks, for culture to grow. Currently no commercially available test has been released for some diagnostic purposes.

Mycoplasma Genitalium as a Pathogen

A lot of paper on Mycoplasma genitalium in NGU have been published. All the studies have shown a higher prevalence and prove of M genitalium in the NGU groups. In addition, Mycoplasma genitalium is detected in men with NGU and negative Chlamydia. Studies on M genitalium to complications such as prostatitis and epididymitis are rare although M genitalium has been found positive on both urethra with epididymitis, and also in prostate glands with prostatitis.

Mycoplasma Genitalium Infections and Antibiotic Therapy

Several types of antibiotics have been used to treat Mycoplasma genitalium infections with certain degree of success. Tetracyclines is discarded but Macrolides, azithromycin has the best chance of cure in men with M genitalium urethritis. Newer quinolones such as moxifloxaci also have effects against Mycoplasma genitalium.

The growth ccyle of Mycoplasma genitalium is longer compared with other bacteria, so a prolonged course of anitbiotics treatment is required in order to eradicate the pathogen completely.


Treatment

When Mycoplasma genitalium is detected, specific treatment for the bacteria is suitable with five days of azithromycin. Single dose of azithromycin may not be effective enough in men with urethritis and resistance may occur. Moxifloxacin treatment for ten days is also suitable if treatment failure after azithromycin


Summary


Mycoplasma genitalium has been agreed as sexually transmitted infection by medical community.

The detection of infection for infection is not known yet.

For cases with symptoms, testing and treatment with Mycoplasma genitalium is advised although accurate tests are not available yet.

The medical comunity now advise the use of azithromycin as first line therapy for Mycoplasma genitalium.

Testing

Mycoplasm genitalium testing methods are very poor, PCR is now used for isolation of Mycoplasma genitalium, but there is an need for more accurate testing method.

Cass with symptomatic NGU should have a test. If treatments of doxycycline is used and no results, then PCR is needed to find out other bacteria. A majority of cases of Mycoplasma genitalium infections with urethritis usually have persistent infections and recurrent of symptoms are often experienced after antibiotics treatment. It is also valid for patients with epididymitis, prostatitis, arthritis from sexually infected.

Treatment Advice

Because of the evidence of current medical community, patients with genital symptoms of Mycoplasma genitalium should test for that and azithromycin treatment.
